I understand that you, Church of Divine Man- Deja Vu Hotline, do not take the place or substitute medical care and that the spiritual counselings and healings are intended to enhance one's spiritual nature only. I understand that any donations I make are gifts to the Church of Divine Man - Deja Vu Hotline.

I understand that this is spiritual counseling and healing, that it is an intangible spiritual benefit and that the healings work with my healing guide, and that I do the healing work;

Church of Divine Man- Deja Vu spiritual counselors only facilitate my healing.

I agree to indemnify Healing Hotline, DejaVu Hotline, Church of Divine Man, and Divine Healing Center.”
